If you have a positive pregnancy test, you can refer yourself to a midwife without the need to see a doctor.

Please let us know of your referral by completing this form. Once we have received your information we will contact you to book an appointment (the appointment lasts approx. 90 mins). Following the booking the midwife will email you direct with relevant information prior to your appointment.
