Our aims at the practice is to be pro active in managing you health and to try and avoid complications in the future.
You will be invited by text for your annual reviews. This should normally fall around your birthday every year. We will try and synchronize your check to include all your long term conditions. We may send you a questionnaire to complete before your appointment to assist the check up.
It is VERY IMPORTANT that you attend this check up so we can make sure your medications are not causing any harm.
You will be able to book the review via online booking or you can call reception to arrange.
We also do blood monitoring for strong medications which your specialist maybe prescribing you. We have to have a shared care agreement with the specialist before we arrange this. This monitoring will then be set up at intervals according to the medications prescribed.
You will be invited for the injection every 3 months but has to be done after a certain date.
We will invite those at risk by the first week of September each year for flu/covid. The timings can vary dependant on guidelines. We normally hold some open clinics which you an WALK IN. The dates and times of these will be advertised nearer the time. Spring covid boosters- Currently we are still providing covid spring booster clinics. We will send those at risk a text to book in online or via reception. Home visits and care homes flu/covid are covered by our Primary care network (PCN)team of pharmacists.
These are normally booked after the babys 6-8 week check up. Depeartment of health do send letters for you to book in. Our team might chase you up if you have not brought your child in on time for these.
You will receive a letter from the cervical screening when you are due your smear test. We also do send texts for you to book this in if our records are correct.
This check up is designed to pick up patients with possible increased risk of Heart attacks. But we also pick up other health conditions at this check too. These checks start at the age of 40 and you will be invited every 5 yrs if you don’t have any conditions diagnosed on previous health checks,
If you have high cholesterol levels diagnosed then your check up for this might be annually. You will offered statins ( cholesterol tablets) if you are at risk depending on all your parameters.
These are generally carried out by our PCN team of nurses and followed up by the community matron.
The Midwife clinics are held every Thursday currently at the practice. The midwives are in charge of booking these clinics. If you find out you are pregnant please let reception know and they will arrange for you to email the midwifery team and they can arrange your booking.
If you require medical advice when the surgery is closed please visit NHS 111. This will guide to the most appropriate service for your problem. In the event of a medical emergency or life threatening situation phone 999 or go directly to the nearest Accident & Emergency Department. Call 111 if it’s less urgent than 999.
